ESSENTIAL JOB FUNCTIONS:

¨       Provides assistance to clients seeking HCEC/HomeBase services and makes referrals to other resources when appropriate.

¨       Assesses client needs through intake reports, which include services requested by the client, the services provided to the client, and other needs that the counselor and client determine.

¨       Provides counseling and education to clients related to housing issues.

¨       Works with challenging client situations that require a comprehensive housing counseling response. Continue reading

Housing Search Guide for People with Disabilities (Reasonable Accommodation) – from Citizens’ Housing and Planning Assoc., CHAPA)

A reasonable accommodation is a change in the rules or practices of a development so that a person with a disability has equal opportunity to apply for and use the housing. Landlords are legally obligated to provide accommodations that are reasonable to a person with a disability. Accommodations must be practical and can’t pose an ‘undue administrative or financial burden’ on the owner. The accommodations cannot alter the main function of the housing. You can request a reasonable accommodation from an apartment manager during the application process if there is something that would prevent you from easily applying due to your disability. Community Based Housing Units in Leominster

The Watermill apartments in Leominster is currently accepting applications for two 2-bedroom affordable accessible CBH units.

They are expected to be ready for occupancy in mid September.

To obtain an application please contact  Caitlin at Wingate management at 978-840-1420.

To be eligible for one of the CBH units the  person(or family member)  must have a disability, not be a DMH or DDS consumer, and not be  in an institution or at-risk of institutionalization.

Lowell Community Health Center – Health Educator/Case Manager for Teen Programs

Summary:

Lowell Community Health Center Teen Programs will hire a part-time health educator/case manager to provide recruitment, facilitation, training, program delivery, and counseling to youth participants. Provide individual case management, counseling and support of participants and families. This position will have a focus on services to immigrant youth from the African communities; an understanding of African culture and heritage and health beliefs is highly preferred. This position also requires a background or knowledge of project coordination. This individual will work closely with the Program Director, Adolescent Health Program Coordinator and Teen Programs staff. Lowell Community Food Assessment – Needs Your Input!

 The Lowell Community Food Security Coalition has been working together over the past few years to discuss, analyze and strategize the effectiveness of our existing food security systems within the City of Lowell.

They are in the midst of an exciting Lowell Community Food Assessment where we are looking at how Lowell residents access food in the community.

If you are a resident of Lowell (or have access to networks of residents), we would greatly appreciate it if you could ask them to complete this short survey to help inform our planning efforts.
